What’s included
- GP examination and completion of your camp medical form, including a review of your full medical history and immunisations
- Blood and urine samples taken in clinic, processed by our partner laboratory
- Copies of all your results, and the signed form ready to submit
Tests we carry out
Based on the usual Camp America (USA) requirements. If your form lists something different, we follow your form.

What to bring
- Your camp medical form
- A summary of your NHS medical notes (full medical history)
- Your full vaccination record: MMR, diphtheria/tetanus/polio, hepatitis A & B, typhoid, chickenpox, COVID-19
- A list of any regular medicines (you can usually take up to 3 months’ supply)
Ask your GP surgery for your notes and vaccination history early. The GP cannot sign the form without them.
